Documents Required

Please note that all documents provided must be originals, or, a photocopy certified to be a true copy by a Justice of the Peace, Solicitor or by the person witnessing the Statutory Declaration.
1. Originals or “certified true copies” of
a. Relevant documents verifying qualifications (ie. University Degree, Diploma, Transcript of Results, or Letter of Recommendation from relevant educational institution)
b. Registration certificate
c. Current practising certificate
d. Verification of current/most recent registration/enrolment/authorisation from Nurse Regulatory Authority must be requested by you to be sent directly to the Registration Board of the Northern Territory
e. Proof of Identity (eg. Drivers Licence, Passport, Birth Certificate)
f. Evidence of Name Change (eg. Marriage Certificate, Divorce Decree or Deed Poll (if applicable))
g. A reference from your last placement of employment
h. A recent character reference (written within the last 6 months)
2. An application and registration fee in Australian dollars (see Payment Options and Schedule of Fees sections of this form)
